Role Overview:
We are seeking a compassionate and skilled Family Nurse Practitioner (FNP) to join our healthcare team in Charleston, WV. This is a hybrid position offering the flexibility of both in-person and remote work. The ideal candidate will deliver comprehensive primary care services, manage chronic conditions, and promote overall patient wellness through a holistic approach.
Key Responsibilities:
- Provide primary and preventive care services to patients of all ages.
- Conduct physical examinations, diagnose conditions, and manage treatment plans.
- Offer counseling and coordinate care for patients with chronic conditions.
- Participate in weight management programs and sexual health services.
- Administer hormone replacement therapy (HRT) and perform Botox treatments.
- Promote a holistic approach to patient health and well-being.
- Maintain accurate and timely patient documentation.
- Collaborate with multidisciplinary teams to ensure quality patient outcomes.
Qualifications:
- Active Family Nurse Practitioner (FNP) certification required.
- Master’s degree in Nursing or related field.
- Minimum 3 years of clinical experience as a Nurse Practitioner.
- Strong knowledge of Primary Care and Urgent Care practices.
- Excellent communication, clinical judgment, and interpersonal skills.
- Ability to work independently and thrive in a hybrid work environment.
- Experience in holistic and comprehensive patient care preferred.
